2nd Option Period: 01/01/2024 - 03/31/2024
3rd Option Period: 04/01/2024 – 06/30/2024

The Request for Quotations No. 75H71023Q00031 using the principles of FAR 12 Acquisition of Commercial Items in conjunction with FAR 13 Simplified Acquisition Procedures will be issued on or by 05/16/2023. Evaluation factors may include:
Required capabilities and performance characteristics:
1. Contractor shall demonstrate:
a) Competence in Pediatrics.
b) Confidentiality of patient care and records.
c) Successful application for credentialing by the medical staff of GIMC.
2. Contractor’s experience of thirty-six (36) months.
3. Contractor’s Licenses/Certifications (ACLS, medical license, Diplomas etc.)
4. Current Board Certified eligibility or certification by the American Board of Pediatrics
5. Completion of an ACGME- Approved Pediatric Residency Program
6. Valid Driver's License
7. Two (2) References of the Contractor
8. Interview with Contractor against Statement of Duties, Performance Work Statement, and Contractor Profile
9. Current Copy of Certificate of Insurance
10. Submittal of Option CLIN
11. Price
Quotes will be due on or by 05/26/2023. The acquisition is a small business set-aside.
