The Indian Health Service, Gallup Service Unit, located in Gallup, New Mexico, 87301 has an upcoming requirement for three (3) Full-Time Non-Personal Healthcare Services for Family Medicine Physician Services in the Family Medicine Department.
The period of performance will be as follows:
Base Period: 04/01/2024 - 06/30/2024
1st Option Period: 07/01/2024 - 09/30/2024
2nd Option Period: 10/01/2024 - 12/31/2024
3rd Option Period: 01/01/2025 – 03/31/2025
The North American Industry Classification System or NAICS code is 621111 with a small business size standard of $16.0 M.
The Request for Quotations No. 75H71024Q00044 using the principles of FAR 12 Acquisition of Commercial Items in conjunction with FAR 13 Simplified Acquisition Procedures will be issued on or by 02/08/2024. Evaluation factors may include:
Required capabilities and performance characteristics:
1. Contractor shall demonstrate:
a) Competence in inpatient and outpatient Internal Medicine or Family Medicine, also in rheumatology, or in nephrology, dermatology, or other certified specialty.
b) Competence in internal and family medicine and certified specialty.
c) Confidentiality of patient care and records.
d) Successful application for credentialing by the medical staff of GIMC.
e) Maintenance of professional certification through participation in appropriate programs for continuing medical and pharmaceutical education.
2. Contractor’s experience of twelve (12) months.
3. Contractor’s Licenses/Certifications (BLS, medical license, Diplomas etc.)
4. Current Board Certified eligibility or certification by the American Board of Internal Medicine or Family Medicine.
5. Completion of Rheumatology, Nephrology, Dermatology and/or other sub-speciality training or fellowship of at least 2-4 years.
6. Valid Driver's License
7. Two (2) References of the Contractor
8. Interview with Contractor against Statement of Duties, Performance Work Statement, and Contractor Profile
9. Current Copy of Certificate of Insurance
10. Submittal of Option CLIN

Quotes will be due on or by 02/20/2024. The acquisition is unrestricited.
