Description

The Indian Health Service, Shiprock Service Unit, Dzilth-Na-O-Dith-Hle Health Center, located in Bloomfield, New Mexico, has an upcoming requirement for the purchase seven (7) Eyewash Stations and installation of the seven (7) Eyewash Stations.



The anticipated period of performance is yet to be determined, not exceed twelve (12) months.



The North American Industry Classification System or NAICS code is 238220 - Plumbing, Heating, and Air-Conditioning Contractors with a small business size standard of $16.5M.



The Request for Quotations No. IHS1462147_1 using the principles of FAR 12 Acquisition of Commercial Items in conjunction with FAR 13 Simplified Acquisition Procedures will be issued on or by March 8, 2023. Evaluation factors may include technical capability to meet the government need outlined in the Statement of Work/Commercial Specifications, professional certifications, past performance, and price. Quotes will be due on or by March 28, 2023. The acquisition is a Unrestricted.



In accordance with 52.204-7(b)(1) prospective contractors must have – and maintain (in accordance with 52.204-13(c) – an active profile in the System for Award Management at beta.SAM.gov in order to submit a quote.



SITE VISIT:



The requesting department intends to have a pre-proposal site visit conducted at DZHC on March 16, 2023 at 10 am (MST) for the purpose of viewing the location details, assessing and determining the needs for the eyewash station installation. Potential Offerors must submit the names of no more than two (2) attendees no later than March 13, 2023 at 3 pm (MST) to the following points of contact:
