**THIS IS A BUY INDIAN SET ASIDE per Buy Indian Act, 25 U.S.C. 47 and only offers from Indian Owned Economic Enterprises will be accepted.

The Indian Health Service, Gallup Indian Medical Center (GIMC) located at 1808 West Aztec Blvd in Gallup NM, intends to issue multiple Blanket Purchase Agreements (BPAS) for various toner cartridges on an “As Needed Basis” for a base period of 12 months and one option year for an additional consecutive 12 months. The North American Industry Classification System or NAICS Code is 334118 – Computer Terminal and Other Computer Peripheral Equipment Manufacturing with a small business concern of 1000. The agency will be placing firm fixed-priced Multi-Award Blanket Purchase Agreements (BPA) with at least three qualified vendors to deliver toner cartridges as found on the attached Price List. Per FAR 6.302-1(c)(2) GIMC has identified specific brand names, but will accept brand name or equal to products.

Any Indian Owned business interested in setting up a BPA for the aforementioned needs are encouraged to submit their capability statement, price list (attached price list form), and catalogs to Gallup Indian Medical Center – Acquisitions Office.

In accordance with: 52.204-7(b)(1) System for Award Management. An Offeror is required to be registered in SAM when submitting an offer or quotation, and shall continue to be registered until time of award, during performance, and through final payment of any contract, basic agreement, basic ordering agreement, or blanket purchasing agreement resulting from this solicitation. 52.204-13(c) System for Award Management Maintenance. The Contractor shall maintain registration in SAM during contract performance and through final payment of any contract, basic agreement, basic ordering agreement, or blanket purchasing agreement. The Contractor is responsible for the currency, accuracy and completeness of the data within SAM, and for any liability resulting from the Government's reliance on inaccurate or incomplete data. To remain registered in SAM after the initial registration, the Contractor is required to review and update on an annual basis, from the date of initial registration or subsequent updates, its information in SAM to ensure it is current, accurate and complete. Updating information in SAM does not alter the terms and conditions of this contract and is not a substitute for a properly executed contractual document.

52.211-6(a) Brand Name or Equal to applies if an item in this solicitation is identified as "brand name or equal," the purchase description reflects the characteristics and level of quality that will satisfy the Government’s needs. The salient physical, functional, or performance characteristics that "equal" products must meet as specified in the solicitation.

Offerors shall submit pricing on the Attached “Price List” and return the completed IHS IEE Representation Form.
