Description



The Indian Health Service, Four Corners Regional Health Center, located at US HWY 160 & Route 35 in Red Mesa, AZ has an upcoming requirement for a Blanket Purchase Agreement (BPA) for the purchase of miscellaneous housekeeping supplies and materials. Symmetry Stealth Dispenser-Prestige, Black, 1250ML; Symmetry GC Foaming Wash, Unscented, 1250ML, Symmetry Foam Sanitizer Fragrance-Free holder; 3.5” base extensions; 16” Uniko Pocket Mop holder w/ clip and locking connector, blue; Velcro system mop holder, blue; Telescopic aluminum handle, 38-72” w/ ergo top, blue; Wiper Chem-Ready refill rolls; Tennant VSMI/VDMU bags with filters; Exhaust filters; Telescopic handle 38-72”, Snake hand duster, Microfiber long shag dusting sleeve, white/purple, Brillante Ultra Microfiber, etc. s for the Housekeeping Department at Four Corners Regional Health Center (FCRHC).



The Period of Performance for this requirement will be for 12 continuous months starting from the Date of Award (i.e. 01/10/2022 – 01/09/2023).



North American Industry Classification System (NAICS) code is 365612 – Polish and Other Sanitation Good Manufacturing with a small business size standard of 750 employees.



The Request for Quotation will be issued on or by October 27, 2022. Evaluation factors may include technical capability to meet the government need, the supply must be readily available when needed, and delivery time of 7-10 business days. Quotes will be due on or before November 3, 2022. The acquisition is Unrestricted.



In accordance with 52.204-7(b)(1) prospective contractors must have – and maintain (in accordance with 52.204-13(c) – an active profile in the System for Award Management at beta.SAM.gov in order to submit a quote.



*There is no SF 1449 attached as the total estimated value is under the Simplified Acquisition Threshold.
